Description	Consists of completed application forms and consent forms for patrons requesting library service. Note: Applications for library service are no longer required for UOIT or Durham College employees or students who have accounts in Banner. Banner data is mirrored, and implied consent to receive library service is assumed. Other patrons, including community members, alumni, and retired staff may still be required to submit application forms.
