Student Sexual Violence Anonymous Disclosure

The ANONYMOUS DISCLOSURE FORM is intended to provide current and past Ontario Tech University students with a confidential channel to anonymously disclose any acts of sexual violence that they experienced on or off-campus for the purposes of data collection. In an anonymous disclosure, the student sharing their experience will remain unknown to the university and will not be asked to share any personal information. 

The information disclosed through the form will be shared with the Human Rights Office who will, if possible, identify patterns of sexual violence (for example, multiple incidents occurring in the same campus location) and take appropriate action to support community safety.

All anonymous disclosure information will remain confidential in accordance with the Student Sexual Violence Policy and Procedures.
